Can anyone show me where there is a form for this so I can print it off and fill it out?
If you don't put a location in your for sale ad that must mean you're willing to deliver.

Good luck. I've been asking for over a year, no one has this form. I've been using white-out and reusing an old one over and over again.

What all does it include? Six lines of species and date, is there a pace for bap chair signature on each species line? Ill just recreate my own and add theMAS logo to it.

Just check the 'Specialization entry' or 'Both(Two bags of fish submitted)' box on the regular bap form. http://aquarium.mn/media/docs/BAP_Entry_Form.pdf

There is a BAP form for specialization. Not sure why it is not on internet for down loading.
Those working on a specialization need this form as you are the only one's keeping track of your specialization entries.
Also each species needs to be signed off by BAP chair or committee member.
Phil is correct in that second bag needs to be noted as specialization on BAP/HAP auction sheet. Do not rely on auction sheet as your specialization record.

Perhaps this issue of missing forms will be brought up at next BOD meeting.
OR
Consider sending an email to BAP chair. Use aquarium.mn list of BOD members and committee chairs to send messages to specific committee chairs. Using this method should get a faster response and a more accurate response.

MAS does not monitor members activities. Stop, start, keep going - it makes no difference to other MAS members. The reason points are awarded and recorded is for members to measure themselves against themselves. Attempting to measure two members involved in BAP just using BAP points is a pretty silly way to compare them.

This form is finally available online. - http://aquarium.mn/programs/bap-breeder ... sion-forms - I have about 12 of these in progress all from using whiteout on an old form and copying it. :lol: Will be nice to start using forms that don't look like they've been run through the washing machine.
